HP Envy 15 Notebook PC - BSOD loop

HP Envy 15 Notebook PC - Windows 8.1

 

I had the below issues when I first got the laptop, somehow got it to boot to the desktop, updated drivers, and everything worked for a few weeks.  I just installed some windows updates and removed McAfee and I am now getting the below issues and can't seem to get back to the desktop:

 

Problem:

- On startup, I see the HP logo and then it goes right to the BSOD "Your PC ran into some problems and needs to restart.  We are just collecting some error info, and then we will restart for you. (0% complete)

SYSTEM_THREAD_EXCEPTION_NOT_HANDLED (iaStorA.sys)

 

- Every other time it goes to the screen "Preparting Automatic Repair .... loading files" before going to the BSOD

 

- I can't boot into any of the Safe Mode options (does the same as above)

 

- I created a recovery USB stick so I could boot into the recovery screen

 

- When I go to Troubleshoot --> Advanced Options --> Automatic Repair --> Windows 8.1  It goes to Diagnosing your PC and then it says Automatic Repair couldn't repair your PC

 

- When I go to Troubleshoot --> Advanced Options --> System Restore and choose the last restore point, it comes back completed, but still boots into the BSOD.  If I pick an earlier restore point, it the restore fails

 

- When I go to Troubleshoot --> Refresh Your PC --> Windows 8.1  It says the drive is locked
